Your Choices
For certain health information, you can tell us your choices about what we share. If you have a clear preference for how we share your information in the situations described below, talk to us. Tell us what you want us to do, and we will follow your instructions.

How do we typically use or share your health information?

Example: A doctor treating you for an injury asks another doctor about your overall health condition.

We can use and share your health information to run our practice, improve your care, and contact you when necessary.
Example: We use health information about you to manage your treatment and services.
Bill for your services
We can use and share your health information to bill and get payment from health plans or other entities.
Example: We give information about you to your health insurance plan so it will pay for your services.
How else can we use or share your health information?
We are allowed or required to share your information in other ways – usually in ways that contribute to the public good, such as public health and research. We will share information about you if state or federal laws require it, including with the Department of Health and Human Services if it wants to see that we’re complying with federal privacy law.

Changes to the Terms of this Notice

NOTE ON STATE LAW: California Confidentiality of Medical Information Act (CMIA)
We are required to comply with California law, including the Confidentiality of Medical Information Act (CMIA), which often imposes greater restrictions on the use and disclosure of your medical information than HIPAA.
In the event of a conflict between the privacy provisions of HIPAA and California law, we will follow the law that provides greater protection to your health information.
This includes, but is not limited to, the following areas that require special care or written patient authorization under state and federal law:
Substance Use Disorder (SUD) Records (42 CFR Part 2): Records relating to the identity, diagnosis, prognosis, or treatment of a patient in a federally assisted SUD program are subject to the strict confidentiality requirements of Federal Law (42 CFR Part 2) and generally require specific written authorization for disclosure, even for many treatment, payment, and healthcare operations purposes otherwise permitted by HIPAA.
HIV/AIDS Test Results: Disclosure of HIV/AIDS test results is highly restricted under state law and often requires specific written authorization from the patient.
Genetic Information: Information resulting from genetic testing has special confidentiality protections under California law.
Minor’s Records: If a minor is authorized by law to consent to their own care (e.g., for mental health, substance abuse, or reproductive services), the minor generally has the exclusive right to authorize the disclosure of that information, and parents/guardians may not have access without the minor’s consent.
Reproductive Health Information: California law provides enhanced privacy and protection for records related to reproductive health services.
